Position Overview

We are seeking a compassionate and dedicated PRN Hospice/Palliative Care Nurse Practitioner to join our team. In this role, you will provide comprehensive palliative care and hospice services, focusing on symptom management, end-of-life care, and support for patients and their families. This position offers flexibility and the opportunity to make a meaningful impact in the lives of those facing serious illness.

Key Responsibilities

  • Conduct comprehensive assessments of patients’ physical, emotional, psychosocial, and spiritual needs.
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams.
  • Provide symptom management and palliative interventions, ensuring comfort and quality of life.
  • Educate patients, families, and caregivers on disease progression, treatment options, and end-of-life care.
  • Coordinate with physicians, nurses, social workers, and other healthcare professionals to ensure integrated care.
  • Complete documentation in compliance with regulatory and organizational standards.
  • Participate in on-call rotations and provide telehealth support, as needed.

Qualifications

  • Master’s or Doctorate degree in Nursing.
  • Current Nurse Practitioner license in the State of Michigan.
  • Certification in Hospice and Palliative Nursing (CHPN) preferred
  • Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred
  • Strong communication, assessment, and critical-thinking skills
  • Ability to provide compassionate care in challenging situations
  • Valid driver’s license and reliable transportation for field visits
